What should I bring with me when I come for an appointment?
When you come in for your appointment, remember to bring the following:
- Your insurance card
- List of current medications you are taking
- Pertinent medical records, operative notes, EMGs, written reports and imaging films/CDs of X-rays & MRIs
Note: If any of the above were provided by a University of Michigan physician or in a University of Michigan center, we will have access to these and you do not need to bring them to the appointment.
- University Health Service (UHS) records are not available to us. If you were seen prior to August 15, 2012, you will need to bring those with you.
- Any forms that need to be filled out by our physician. Your portion of the form should be completed prior to arriving for your appointment.
- Athletic shorts, comfortable clothing, and tennis shoes for lower extremity evaluations (and in case you need to do Physical Therapy while you are here). If you are a female patient being seen for a shoulder problem, please bring a tank top.
Note: We have full locker room services.
What should I expect during my first visit?
During your initial visit, our providers will take your medical history and conduct a comprehensive physical examination and evaluation of your problem and symptoms.
Your injury or condition will be completely assessed for range-of-motion, pain, swelling, abnormality, instability, and strength. We want to know the severity of your pain and injury, as well as how it interferes with your participation in sports, daily activities, and work.
Your provider will review any previous imaging or tests you had, discuss your injury or problem, make a diagnosis, review options, and recommend treatment.
If additional diagnostic imaging—such as X-rays or an MRI—is needed to help make a diagnosis, these studies will be performed at a time and location convenient for you. X-ray services are available onsite, and MRI studies are performed at East Ann Arbor, Brighton Center for Specialty Care, & Northville Health Centers, as well as the C.S. Mott Children’s and University Hospitals.
